What Do I Need To Complete The Application Process?

The application process for each person may vary widely in terms of time and necessary documentation. The list below are possible items or information you may want to gather to assist in speeding up your application process. You'll want to talk with your counselor or contact person at DSB to determine which, if any, of these items are necessary.

• Photo ID
• SSI/SSDI Award Letter
• Medical Documentation on Eye Condition
• Medical Provider's Contact Information
• DVR Counselor Name and Contact Information (if appropriate
